Job Summary

The Pharmacist is responsible for the safe and effective dispensing of prescription medications, as well as providing comprehensive pharmaceutical care to patients. They work closely with healthcare providers, patients, and other members of the healthcare team to ensure optimal medication management and positive patient outcomes.

Key Responsibilities

– Receive, review, and validate prescriptions to ensure accuracy and appropriateness
– Interpret and analyze patient health information, medical records, and clinical data to identify potential drug-related issues or interactions
– Prepare, package, and dispense prescription medications, ensuring compliance with all applicable laws and regulations
– Provide patient counseling on the proper use, storage, and potential side effects of prescribed medications
– Monitor and evaluate patients’ response to medication therapy, and make recommendations for adjustments or changes as needed
– Collaborate with physicians, nurses, and other healthcare providers to develop and implement comprehensive medication management plans
– Maintain accurate and detailed records of all prescription-related activities, including inventory management and controlled substance handling
– Participate in the development and implementation of policies, procedures, and quality assurance initiatives related to medication management
– Stay informed of the latest developments in pharmacology, therapeutics, and relevant legislation or regulations
– Contribute to the education and training of pharmacy technicians, interns, and other healthcare professionals
– Serve as a subject matter expert and resource for patients, caregivers, and other healthcare team members

Required Qualifications

– Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy
– Current licensure as a Registered Pharmacist (RPh) in the applicable state(s)
– Minimum 2-3 years of experience as a practicing pharmacist, preferably in a healthcare setting
– Strong clinical knowledge and expertise in pharmacology, therapeutics, and medication management
– Excellent interpersonal, communication, and problem-solving skills
– Proficient in using pharmacy-related software, databases, and information systems
– Ability to work collaboratively with diverse healthcare teams and prioritize patient care
– Commitment to professional development and staying up-to-date with industry best practices

Preferred Qualifications

– Board certification in a relevant specialty (e.g., Ambulatory Care Pharmacy, Critical Care Pharmacy)
– Experience in a hospital, clinic, or long-term care facility setting
– Expertise in a specialized area of pharmacy practice (e.g., oncology, infectious disease, geriatrics)
– Involvement in professional organizations or leadership roles
