SURVEILLANCE AGENTS FOR TUTUME DISTRICT (7 POSITIONS – TUTUME X 1 , NATA X 2, GWETA X 1, DUKWI X 1, SEPAKO X 1, MARAPONG X 1)

The above subject matter refers;

The District Health Management Team invites applications from individuals for the above-mentioned post.

KEY RESPONSIBILITIES
1. Reports to the district surveillance officer.
2. Visits Health facilities which include the following types, clinics, HP, Private Doctors, Hospitals
3. Check with the sister/nursing assistant in the clinic their register to make sure there were no cases.
4. Check the status/availability of case investigation kit (RDT and drugs etc)
5. Check whether the facility has enough IEC (malaria awareness posters) material displayed and if not put up at strategic places.
6. Picking up samples at health facilities
7. Identify potential mosquito breeding spaces and map them using GIS
8. Participates in Vector control activities (IRS, LLIN distribution and larviciding)
9. Participates in case investigation
10. Check the malaria notification book to tear out new notifications
11. Educating the healthcare workers to properly manage inventory of malaria commodities and reporting promptly on all identified cases
12. Planning and conducting active case investigation and active case detection
13. Communicating with the communities with potential outbreaks.

QUALIFICATION
• A minimum of Form 5 is required .The applicant must be able to read and write in both Setswana and English (Please attach certified copies of certificates, ID and references)

REQUIREMENTS
1. Must be 18 – 45 years of age
2. Proven ability to perform to accurate work in a short time frame and under minimal supervision
3. Fluent in English
4. Experience in any malaria activities would be an added advantage
5. Applications should indicate the village the applicant is applying from and an affidavit from kgosi should be attached.
6. The applicants should be from the above-mentioned villages

DURATION

1ST March 2019 to 31 August 2019

SALARY SCALE: B5 (P2 469,00) per month
