COMMUNITY HEALTH WORKER

Job Requirements

– BGCSE & Certificate in HIV testing
– 1 Year relevant work experience
– Excellent communication & interpersonal skills.

Key Responsibilities

 1. HIV testing, care & Treatment

– Conduct targeted HIV testing and counselling using both direct and index testing approaches.
– Mobilize communities through disclosure campaigns and client tracing
– Enroll newly diagnosed, treatment-interrupted, and virally unsuppressed clients into community care.
– Monitor and trace missed appointments or defaulters and re-engage them in care.

2. TB and PMTCT Integration

– Enroll eligible clients in community ART medication refills and coordinate deliveries with facility teams.
– Provide community-based TB care (CTBC) including DOT, contact tracing, and screening
– Identify HIV-exposed infants and ensure testing completion through PMTCT service linkages.

3. Data Management, Quality Control, and Reporting

– Submit weekly and monthly performance reports.
– Accurately complete all manual and digital tools; sync data daily to DHIS.
– Conduct basic stock control (e.g. HIV test kits) and use standard SOPs in all service areas.

4. Community Mobilisation and Coordination

– Collaborate with Senior Community Health Worker (SCHW), facility teams, and local leaders to ensure full-service coverage.
– Support demand creation, tracing, and follow-up campaigns in assigned catchment areas.

5. Any other reasonable and lawful requirements

– Incorporate Collaboration, Learning and Adapting (CLA) into all levels of programming to inform program design and implementation.
– Be available for weekend or after-hours work as required.
